The Couch to 5K Program has come to a close (as of 5/8/21)! A huge congratulations to all staff who have participated in this event! Give yourself a little victory dance!
To earn the 100 points towards the wellness goal, please send in your completed tracking sheet to Kayla Hackett, in Human Resources, by this Friday, May 14, 2021. The tracking sheet can be found on page #5 in the Couch to 5k attachment below. To submit your completed tracking sheet, please scan/email it to firstname.lastname@example.org OR fax it to HR at (570)917-8091. A tracking sheet must be submitted in order for you to earn the points toward the wellness program!
It is requested of all supervisors/managers to ensure that staff properly receive all distributed information related to these programs so all eligible staff have an opportunity to participate.
ALL ACTIVITIES/SCREENINGS MUST BE COMPLETED BY MAY 31, 2021 TO RECEIVE THE POINTS FOR THE $520 ANNUAL SAVINGS ON THE HIGHMARK HEALTH INSURANCE PLAN!
In addition to the event mentioned above, make sure to complete events outlined in the brochure to reach the 1,000 point goal. Those who reach this point goal will receive the wellness discount on the Highmark health insurance plan this next plan year (effective 8/1/2021). Attached below is the program brochure for the Wellness Program that portrays a list of online activities you can complete through the portal to earn the points necessary. If completing any preventative screenings with a doctor, eye center, or dentist, you can get points for those as well! Forms for physicals & preventative screenings can be found on the HOPE portal, as well as all of the information attached above.
